Registering a Death

After the loss of a loved one, the death will normally be registered at the local Registrar's office. There are other ways in which a death may be registered and we will advise on these, when appropriate. You have five working days to register a death, although there are special circumstances in which exceptions can be made. For recording a death with the local Registrar's office, the following information and documents are required:

  • Medical Certificate of the Cause of Death
  • Medical Card
  • Birth Certificate and/or Marriage Certificate

Although the above information is useful to the Registrar, it is not essential provided you can answer the following:

  • Date and place of death
  • Full name and surname of the deceased (and maiden name if deceased was a married woman)
  • Date and place of birth
  • Occupation (and if deceased was a married woman or widow, the occupation of the husband)
  • Usual address of the deceased
  • Whether the deceased was in receipt of a pension or other public funds
  • If the deceased was married, the full name and date of birth of the surviving widow

Information on Local Registrar's Offices

Copies of the Entry of Death (Death Certificate) may be obtained from the Registrar's office upon payment of the necessary fee. Banks, building societies and insurance companies may only accept the Registrar's Copy of the Entry of Death as opposed to a photocopy. The Registrar will provide you with the following:

Certificate of Burial or Cremation (a green form) – this MUST be delivered to your funeral directors as soon as possible prior to the funeral taking place.

Certificate of Registration of Death (a white form) – this should be completed and forwarded along with deceased's Pension/Benefit books to the local Department of Social Security.

The work of the Coroner

The Coroner's role is to establish the cause of death when a doctor is unable to produce a Medical Certificate of the Cause of Death. If a Coroner is not involved in the process, cremation forms will need to be completed by third party doctors or another independent doctor. If the death occurs in a nursing home or domestic home. We can assist with this.

Procedures for registering a death will differ when the Coroner becomes involved, so if you require additional advice, please contact us.
